Repeat Prescriptions Form
Ensure that you have checked with your Pharmacy that you don’t have any more repeats still to be fulfilled. If you run out of medication and urgently need a prescription, we may only be able to prescribe a short-term supply. For a longer-term prescription or for some medication, you may need to see the Doctor or Nurse Prescriber.
There is a fee of $15 for Repeat Prescription requests.
Requests will be processed within 48 - 72 hours of receipt. To enable us to fulfil your request promptly, please fill out the form with the complete information. If further information is needed, we will give you a call.
